Dr. Nellgård underlined the importance of performing an early cricothyrotomy in a situation of CICV. After several failed attempts to intubate a patient with instable angina pectoris scheduled for coronary artery bypass surgery, bag and mask ventilation became unsuccessful.
A transtracheal catheter (2 mm I.D.) was placed and ventilation with Ventrain increased saturation from <50% to ~80%. Then, cricothyroidotomy was successfully performed using the Melker Cricothyroidotomy 5. Surgery was postponed and percutaneous coronary intervention was performed instead.
